In order to begin the admissions process to LUTHERAN HEALTHCARE CENTER for Skilled Nursing, Rehabilitation Services, or the Jeppson Residential Care Center, please request and complete an application.  To obtain an application, stop by LUTHERAN HEALTHCARE CENTER or contact our Director of Admissions at (508) 754-8877 ext. 235 or by email at admissions@lhcare.org.  You may also download the application here.

Once we have received a completed application, it will be reviewed.  After the approval of an application, staff will contact the applicant and/or designated family members to discuss arrangements for admission.  All admissions are based on room availability.

Applicants who are eligible for Medicaid, Medicare or private insurance coverage may call to learn more about its benefits for residents.  Admissions may be arranged from any area hospital or from the community.  Transfers from other nursing homes may also be arranged.

Fees for Residents

Traditional Skilled Nursing Care:

Private Room:	$288/day
Double Room:	$278/day
Triple Room:	$262/day

Jeppson Residential Care:

Private Room:	$115/day
Double Room	$110/day

Daily rates include standard residential services, three-meals/ day, snacks, housekeeping, laundry, social activities, and cable television.
