In order to begin the admissions process to the Lutheran Healthcare Center for Skilled Nursing, Rehabilitation Services or the Jeppson Residential Care Center, please request and complete an application. To obtain an application, stop by the Lutheran Healthcare Center or contact our Director of Admissions at 508-754-8877, ext. 235 or by email. You may also download the application here. Once we have received a completed application, it will be reviewed. After the approval of an application, staff will contact the applicant and/or designated family members to discuss arrangements for admission. All admissions are based on room availability. Applicants who are eligible for Medicaid, Medicare, or private insurance coverage may call to learn more about its benefits for residents. Admissions may be arranged from any area hospital or from the community. Transfers from other nursing homes may also be arranged. Fees for Residents Traditional Skilled Nursing Care:

Daily rates include standard residential services, three-meals a day, snacks, housekeeping, laundry, social activities, and cable television.
